Grants of up to $30,000 to South Dakota cancer treatment centers for the implementation of evidence-based programs that improve the quality of life for survivors of cancer. Funding is intended for programs with a lasting impact that have a focus on a permanent culture change around cancer survivorship.

Funds can be used for items such as:
- Staff time for informatics/data analysis, developing and implementing policies and workflows, implementing reminders, provider interventions, navigation, and key project initiatives.
- Development and mailing costs for reminders or automated reminder costs.
- In-state travel to support intervention implementation (at state rates; see budget instructions in Appendix A).
- Other eligible expenses. Please contact program staff with any budgetary questions.

It is anticipated that two applicants will be funded.

Estimated Size of Grant:

Applicants may request up to $30,000.

Project Period:
Year One: July 1, 2019 – May 31, 2020
Subsequent Years: Additional funding may be awarded for two additional years based on performance and availability of funds.

Additional Eligibility Criteria:

Eligible applicants include cancer treatment centers located within the geographical borders of South Dakota.

Funding Restrictions:
- Funds cannot cover any type of direct service (i.e. direct delivery of care). Direct services may be covered based on your standard organizational practice (i.e. paid for by insurance, private pay, etc.).
- Funds may not be used for research activities, lobbying efforts at the local, state, or federal level or for the purchase of food, beverages, equipment, or client/patient/provider incentives.
- Funding will be awarded to an organization only and not to an individual(s).

Pre-Application Information:

Staff will be available to answer questions, review grant applications, and provide recommendations for revisions until May 28th.

Application Deadline: 5:00pm CT on Wednesday, May 29, 2019

Notice of grant award issued (via email): Week of June 3rd (tentative)

Funding will be remitted on a reimbursement basis.

Technical assistance is available to all interested applicants throughout the application period. This includes, but is not limited to, discussing application plan, addressing questions, reviewing draft applications, and providing feedback.
